Timeless Facial Renewal

Introduction to Facelift in San Antonio

Aging of the face and neck is a complex process that involves many factors. Over time, our tissues lose their strength and elasticity, and as skin loses elasticity, gravity causes sagging skin and our tissues to descend lower in the face. There are also complex changes in facial volume that influence our facial appearance. The result can be a deepening of the smile-folds around the mouth, development of jowls along the jawline, and visible excess skin under the chin (the “turkey waddle”).

Our gender, age, genes, body weight, bone structure, skin quality, and overall health all affect the rate and extent of this aging process. These natural processes can be accelerated by excessive sun exposure, smoking, and medical conditions like diabetes, not to mention fluctuations in body weight. Each of these areas can be addressed with a facelift procedure offered here in San Antonio, TX, to help restore a more youthful appearance.

What is a Facelift?

Historically speaking, the term “facelift”, or rhytidectomy, was a misnomer because it primarily referred to surgery targeting the lower third of the face and upper neck. Treatment of the cheeks, eyes, and forehead required additional procedures. Today, face lifting with Dr. Stallworth is a surgical procedure used to reposition, elevate, and tighten the underlying support layers of the midface (cheek), lower face, and neck, including the underlying deeper facial tissues. This is most often performed with the modern techniques of deep plane lifting and structural contouring of the deep neck as well. The goals include removal of the jowling, improvement of the jawline and upper neck, removal of excess skin if necessary, and a rejuvenated appearance that preserves natural facial features. When indicated, this is the most direct and dramatic method of rejuvenating the cheeks and lower face and is the reason why it’s one of the leading facial plastic surgeries in America, with over 79,000 performed just in 2024, according to the American Society of Plastic Surgeons.1

Dr. Stallworth performs most facelift procedures under general anesthesia in the operating room on an outpatient basis. During your consultation and preoperative visit, you will learn about the goals of your surgery, the techniques that will be used, the surgical process, your recovery, and the risks of surgery.

Facelift surgery is not a one-size-fits-all procedure. Depending on the degree of facial aging, anatomy, and aesthetic goals, Dr. Stallworth may recommend several different approaches.

Not everyone needs a full facelift. For individuals who are beginning to notice early jowling, mild skin laxity along the jawline, or subtle changes in facial definition, a mini facelift may provide meaningful improvement with a less extensive surgical approach.  A mini facelift helps to correct the earliest signs of tissue descent before they progress into more advanced facial aging. However, the power and longevity of mini-lifting is limited and often not appropriate for many patients.  Mini lifts cannot address excessive skin laxity, do not offer an option for deep neck sculpture, and are overall limited in scope.

What Are the Benefits of a Facelift?

One of the most common misconceptions about facelift surgery is that it simply tightens loose skin. In reality, a modern facelift addresses facial skin, underlying tissues, and deeper support structures rather than only tightening loose skin. By repositioning deeper tissues, restoring definition to the jawline, and removing excess skin when necessary, a facelift can improve multiple signs of aging simultaneously. Some of the benefits that come from this surgery are that it:

  • Restores definition to the jawline by reducing jowling
  • Improves loose skin and tissue laxity in the lower face and upper neck
  • Creates a smoother transition between the face and neck
  • Produces long-lasting rejuvenation that cannot be replicated with injectables and mini-lifting techniques alone
  • Helps patients look more rested, vibrant, and refreshed, supporting a refreshed appearance that preserves natural beauty
  • Allows for the sculpture of deeper facial anatomy including fat, muscle, and salivary glandular tissue
  • Provides a facial rejuvenation that keeps people looking like their younger selves rather than someone else

Who Are the Best Candidates for a Facelift?

People often assume a facelift is reserved for individuals of a certain age, but chronological age is far less important than the degree of facial aging and an individual’s health. While ideal facelift patients are often in their 40s to 60s, some begin developing significant jowls and neck laxity in their forties due to genetics, while others maintain strong facial definition well into their 60s and 70s. The question is not necessarily “How old are you?” but rather “What changes are you seeing in the mirror?” At Texas Plastic Surgery,

You may be an excellent candidate in the San Antonio area if you identify with the following characteristics. Good candidates are generally in good health, non-smokers, non-diabetic, not requiring blood-thinning medications, near a stable optimal weight with a BMI of 30 or less, and:

  • Have good skin quality
  • Bothered by jowls or loss of jawline definition
  • Experiencing loose skin in the lower face or neck
  • Have neck fullness from fats, glands, and muscle bulk that is not responsive to weight loss and too deep for liposuction
  • Seeking long-lasting facial rejuvenation
  • Have outgrown the benefits of non-surgical treatments
  • Have realistic expectations regarding surgical outcomes

Combining My Facelift with Other Cosmetic Procedures

Facial aging rarely affects only one area. While a facelift can significantly improve the lower face and upper neck, many patients choose complementary procedures at the same time to achieve more comprehensive rejuvenation. A brow lift can enhance the upper face, and chin augmentation may improve overall facial balance when paired with a facelift.

The neck is often one of the first areas to show visible signs of aging, and a neck lift focuses on tightening and contouring the neck to complement facelift results. Loose skin, muscle banding, and excess tissue beneath the chin can persist even after a successful facelift if they are not directly addressed. Combining a neck lift with a facelift allows Dr. Stallworth to improve the jawline and neck as a unified aesthetic unit, often producing one of the most dramatic rejuvenation effects available in facial surgery.  In addition, for younger patients who do not have skin laxity, but rather have submental fullness from fat, muscle, and glandular tissue that is not amenable to weight loss and exercise, a deep structural neck lift may be a great option.

How to Begin Your Facelift Journey

For many patients, the decision to explore facelift surgery does not happen overnight. It often develops gradually over years, with changes noticed in photographs, video calls, or daily interactions with the mirror. Some become frustrated by the appearance of jowls despite maintaining a healthy lifestyle. Others feel that their neck has begun aging faster than the rest of their face. And more recently, patients taking control of their health through weight loss are noticing more rapid and significant facial and neck volume and structural changes. The consultation process in San Antonio provides an opportunity to determine what is truly causing these concerns and to identify solutions most likely to address them.

  1. Participate in a comprehensive facial aging assessment
  2. Discuss your concerns, motivations, and desired outcomes
  3. Review your medical history, medications, and lifestyle factors
  4. Learn about surgical options that may be appropriate for your anatomy
  5. Receive a customized treatment plan and detailed cost estimate

What to Expect On the Day of Facelift Surgery

Patients often arrive on surgery day feeling a mixture of excitement and nervous anticipation. Before the procedure begins, Dr. Stallworth reviews the surgical plan, performs final markings of the face and neck, and answers any remaining questions. This is an important part of the process because every facelift is a cosmetic surgery procedure customized according to the patient’s facial anatomy and rejuvenation goals.

Once anesthesia has been administered, the procedure focuses on repositioning the deeper support structures responsible for facial aging while preserving natural facial features. This is done with strategically placed incisions that allow access to the deep structures of the face and neck. These tissues are then augmented and secured into their previous positions. Excess skin is then conservatively removed so the results appear natural rather than stretched, and the incisions are meticulously repaired. Typical procedures may take four to eight hours to complete depending on the extent of surgery required and the addition of other complementary procedures.

Following the operation, patients spend time in recovery before returning home or to a local hotel the same day. Detailed instructions regarding activity restrictions, sleeping position, incision care, swelling management, and medications are provided to support a smooth recovery with minimal discomfort. Overnight nursing care is also an option for patients who desire this level of care. All patients begin the follow-up process in the clinic first thing the next morning.

Facelift Recovery

Your Guide to Post-Facelift Care

One of the most important things for patients to understand about facelift recovery is that healing occurs in stages. The first week is focused primarily on rest, swelling management, and allowing the tissues to begin settling into their new position. When post-operative instructions are followed closely, recovery tends to involve minimal discomfort for most patients. The second and third weeks are often when patients begin to feel significantly more like themselves. Bruising starts to fade, swelling becomes less noticeable, and many individuals feel comfortable returning to work, social activities, and public settings.

However, healing is still actively occurring beneath the surface. The deeper tissues that were repositioned during surgery require time to stabilize and continue to settle during healing, and subtle changes will continue to evolve even after patients feel largely recovered.

Perhaps the most rewarding aspect of facelift recovery is watching the results evolve over time. As residual swelling resolves, many patients gradually develop a refreshed appearance rather than looking dramatically different, and the final refinements can continue for several months, particularly in the neck and lower face. Even so, the natural aging process continues over time, which is why healthy habits and ongoing skin care best support long-term results.

  • “highly recommend her for any plastic surgery you may need”

Dr Rokosz is one classy person!

She listens patiently, tells you what you need to hear and does not waver in her stance on things. I asked her opinion on outcome to expect for a procedure i was entertaining to get and I was disappointed in what she said. She understood that it wasn’t what I wanted to hear but she held her ground and was empathetic. It’s hard to want one thing but reality is not always the outcome we want!

She never makes you feel pressured into any kind of decision and is supportive in helping you figure out what will be best for you! I just love her and highly recommend her for any plastic surgery you may need. I am 3 weeks PO from an arm lift and breast lift …both are doing quite well! As i said ..one classy lady!!
